Salary: $60-80+/hr based on experience LaSante Health Center is seeking an Optometrist to provide comprehensive eye care services in a federally qualified health center (FQHC) setting on per diem basis. This per diem role focuses on delivering high-quality vision care while collaborating with primary care and specialty teams to support overall patient health. Responsibilities: + Perform comprehensive eye examinations and vision assessments + Diagnose and manage common eye conditions and vision disorders + Prescribe corrective lenses and contact lenses as appropriate + Identify and refer patients with complex or surgical needs to specialists + Educate patients on eye health, preventive care, and treatment plans + Document all encounters accurately in the electronic health record (EHR) Qualifications: + Doctor of Optometry (OD) degree from an accredited program + Current New York State optometry license (or eligibility) + Experience in outpatient or community health settings preferred + Strong communication and patient engagement skills
