Oncology Screening Nurse Navigator--Lung Cancer Focus

Full Time – Day Shift

Hamburg Location, Lexington, KY

The Oncology Pathway supports patients throughout the cancer care continuum by coordinating services across diagnosis, treatment, and follow-up care. The department works collaboratively with multidisciplinary clinical teams to ensure timely access to care, evidence-based treatment planning, and a patient-centered experience that promotes quality outcomes and continuity of care.

Job Description:

Assesses patient’s educational, physio/psychosocial needs, barriers to care and support systemsFacilitates information gathering and analysis for patients to make informed decisionsCollaborates with multidisciplinary team members to facilitate timely access and continuity of care across various care settingsDevelops and drives initiatives to eliminate barriers to careFacilitates access to healthcare and community resources for patients as necessaryIndividualizes the care of lung cancer patients throughout the continuumEncourages patients to maintain records of their health historyFacilitates high quality, timely, and individualized patient care through coordination, collaboration, and advocacyMaintains necessary information systems and processes to promote effective development, management, and reporting of the lung screening/cancer programServes as a liaison between the organization, patients, families, physicians, and the community for lung cancer care

Requirements:

Current KY or Compact State License as a Registered NurseBachelor of NursingCertification in Oncology Nursing (OCN, AOCN, AOCNS or AOCNP) within one year of employmentAt least 2 years of oncology experience
