Major Responsibilities:
Coordination and implementation of strategies to ensure quality, efficacy, and cost effectiveness for the oncology patients in multiple settings.Assesses patient needsOrientates patient to the hospital and outpatient settings as well as the organizationThe PONN delivers cancer care across the continuum, from diagnosis through treatment. and implements the transition to survivorship, and end-of-life careUsing the nursing process, the PONN provides patient and family insight with appropriate education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuumCritically thinks and clinically assesses patients providing support and education. Manages the complexity of the cancer diagnosisCommunicates and collaborates with all multidisciplinary team membersReviews distress screening for referrals

Global perspective of changing health care environmentAbility to prioritize workBasic computer skillsEffective interpersonal communication skillsCurrent RN license in the state practicingCPR certificationOCN Certification or Nurse Navigator Certification obtained within 2 years of hire.Physical Requirements and Working Conditions:
Schedule Information: M-F 8-4:30, days/hours flexible based off of provider/clinic schedules to best meet the needs of patients
