**Job Summary and Responsibilities**
As a Navigator RN, you will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys.
Every day you will assess patient needs, educate on treatment options, coordinate care across multiple providers, and facilitate access to resources, acting as a central point of contact.
To be successful, you will demonstrate strong clinical knowledge, exceptional communication and problem-solving skills, and a compassionate, patient-centered approach to ensure seamless and informed care.
+ Provides coordination of care and support for patients, families and caregivers throughout the continuum of care.
+ Provides education for patient, caregiver and family on disease process, plan of care, treatment, symptom management and survivorship.
+ Effectively communicates with patient, caregiver, families and multidisciplinary healthcare teams.
+ Provides nursing assessment through a process of gathering pertinent patient's medical history and review of presenting symptoms.
+ Demonstrates professionalism in the workplace with patients, families, healthcare team and community.
+ Provide excellent customer service resulting in patient satisfaction with care.
**Job Requirements**
**MINIMUM**
+ Bachelors of Nursing Nursing (BSN)
+ 3 years of experience as a RN in an inpatient or outpatient setting
or
+ 1 year RN with 3 years experience coordinating patient care, navigating the healthcare system, providing psychosocial support to cancerpatients, and healthcare administration.
+ Computer skills, EMR documentation,Word, Excel, Record/Verify system
+ Registered Nurse: AZ
+ American Heart Association Basic Life Support - CPR upon hire
**PREFERRED**
+ Oncology experience

Dignity Health Medical Group (DHMG) employs approximately 400 providers and 1200 support staff that cover a wide variety of specialties. **Pay Range**
$35.62 - $52.99 /hour
