Nurse Supervisor Home Care 

 Responsibilities:

 Completes initial comprehensive and ongoing assessment of patient and family to determine best services to meet their needs Coordination of services with the patient, family, and care giver team. Ongoing supervision of all professional and para-professional staff assigned to the patient. Ongoing communication with patient and family to ensure client satisfaction and best outcomes

 Requirements:

Registered Nurse (RN) license NYS BSN required 3 + years of clinical experience Home care experience Minimum 1 year of supervisory experience Excellent observation, problem solving and communication skills Compassion and integrity
