Homeward Bound is hiring a dedicated Nurse Case Manager to support individuals with developmental disabilities in leading full and dignified lives. This position blends clinical practice with leadership and advocacy in a person-centered environment. As a Nurse Case Manager, you will coordinate and oversee the healthcare of individuals served by Homeward Bound. This includes developing health care plans, facilitating care with medical providers, ensuring compliance with health regulations, and mentoring direct care staff.
