Job Description

The Nurse Care Manager provides professional, patient-centered nursing care in the ambulatory setting, adhering to the multi-disciplinary plan of care and established policies and procedures under the supervision of the Lead Nurse. The role involves providing direct nursing and telephonic services to patients and families according to the standards of Ambulatory Care Nursing. The position requires collecting and analyzing patient data, assisting with the development and maintenance of care plans, and evaluating outcomes of interventions as indicated by the multi-disciplinary care plan. Documentation and communication of these interventions and outcomes are performed according to professional and organizational standards. The Nurse Care Manager will also delegate tasks to other licensed and unlicensed care providers as appropriate and ensure compliance with regulatory body standards.

Responsibilities

Utilize assessment skills to evaluate patient needs and formulate a care plan as part of the multi-disciplinary care team. Revise outcomes and interventions based on ongoing evaluations, collaborating with both the patient and the care team.Facilitate individual or group nursing visits with patients to address chronic illnesses and provide education on disease management and treatment as needed.Identify patients at risk for poor outcomes and provide additional outreach and frequent follow-up.Provide complex case management, including chronic disease case management, critical diagnosis care coordination, transition care management, high-risk clinical tracking, complex medication management, and system utilization.Conduct medication reconciliation and adherence counseling, assessing patient knowledge and barriers to adherence and self-care.Triage patients during walk-in/same day clinic visits.Collaborate with the multi-disciplinary team to address both physical and psychological aspects of health.Work with the Transition of Care Team to ensure safe and effective transitions of care.Assist patients in developing self-management goals and coordinate follow-up as needed.Access appropriate resources to meet patient needs and demonstrate awareness of cultural, ethnic, gender, sexual orientation, and racial differences.Teach patients self-care skills, including medication administration and wound care.Delegate clerical and non-nursing clinical tasks to assistive personnel as appropriate.Utilize interpreter services for patient communication as necessary.Attend nursing departmental and site staff meetings, and participate in committee meetings.Document patient communications and health status data continuously and systematically.Complete reviews of systems, physical assessments, and correlate with patient health history.

Essential Skills

Experience with documentation in an Electronic Medical Record.Ability to work with computers in word processing and database applications.CPR certification is required by the scheduled start date.Flexibility and adaptability to a changing environment.Ability to prioritize competing responsibilities.

Additional Skills & Qualifications

Graduate from an Accredited School of Nursing; Current Maryland Nursing License.Preferred: One year of nursing experience in an ambulatory or acute care setting.Bachelor of Science in Nursing (BSN) is preferred.Significant related experience with populations served may substitute for the above requirement.

Work Environment

The work environment is located in the heart of Mt. Vernon, situated in the historic Monumental Life Building. The center provides services in Pediatrics, Adult Primary Care, OB/GYN, and more. The position offers a Monday to Friday schedule with no weekend hours. The facility supports the uninsured and low-income community through grant funding, and offers exposure to multiple specialties, with opportunities for temporary to hire (TTH).

Job Type & Location

This is a Contract position based out of Baltimore, MD.

Pay and Benefits

The pay range for this position is $38.00 - $42.00/hr.
