Job Title: Nurse Care Coordinator

Job Description

The Nurse Care Manager is responsible for providing professional, patient-centered nursing care in an ambulatory setting. This position involves direct nursing and telephonic services to patients and families, adhering to standards of practice for Ambulatory Care Nursing. The role includes collecting and analyzing patient data, developing and maintaining care plans, and evaluating the outcomes of interventions. Communication and documentation of interventions and outcomes are essential, as well as delegating tasks to other licensed and unlicensed care providers.

Responsibilities

Utilize assessment skills to evaluate patient needs and formulate a plan of care as part of the multi-disciplinary care team.Revise outcomes and interventions based on ongoing evaluations, collaborating with patients and care team members.Facilitate individual or group nursing visits to address chronic illnesses and provide disease management education.Identify patients at risk for poor outcomes and provide outreach and follow-up services to this population.Offer complex case management including chronic disease case management, critical diagnosis care coordination, transition care management, and complex medication management.Provide medication reconciliation and adherence counseling, assessing patient knowledge and barriers to adherence.Triages patients during walk-in/same day clinic visits.Collaborate with the multi-disciplinary team to address both physical and psychological aspects of health.Work with the Transition of Care Team to ensure safe and effective transitions.Collaborate with providers during appointments for education on health promotion and disease management.Assist patients in developing self-management goals and coordinate follow-up as needed.Access appropriate resources inside and outside the organization to meet patient needs.Demonstrate awareness of cultural, ethnic, gender, sexual orientation, and racial differences impacting patient outcomes.Teach patients skills for self-care, including medication administration and wound care.Delegate clerical and non-nursing clinical tasks to assistive personnel as appropriate.Utilize interpreter services for patient communication when necessary.Attend nursing departmental and site staff meetings, and participate in committee meetings.Document communications and health status data systematically and thoroughly.Complete review of systems and physical assessments to prepare patients for further examination or treatment.

Essential Skills

Registered Nurse (RN) license.Experience in case management and managed care.Proficiency in patient education and referrals.Knowledge of electronic health records.Experience in acute care and utilization management.CPR certification.

Additional Skills & Qualifications

Graduate from an Accredited School of Nursing.Current Maryland Nursing License.Preferred: One year of nursing experience in an ambulatory or acute care setting.Preferred: Bachelor of Science in Nursing (BSN).Experience with documentation in an Electronic Medical Record.Ability to work with computers in word processing and database applications.Flexibility and adaptability to a changing environment.Ability to prioritize competing responsibilities.

Work Environment

The Mt. Vernon Center is located in the heart of Mt. Vernon, offering a full range of services including Pediatrics, Adult Primary Care, and OB/GYN. The position follows a standard Monday to Friday schedule with no weekends. The historic Monumental Life Building houses the downtown site, providing exposure to multiple specialties.

Job Type & Location

This is a Contract position based out of Baltimore, MD.

Pay and Benefits

The pay range for this position is $40.00 - $45.00/hr.
