**17559BR** **Title:** NP / Physician Assistant **Department/Cost Center:** 710 - Palliative Care **Job Description:** 50-60%Office Practice + See 4-8 patients in each half-day care session. 6 – 4 hour half day sessions per week depending upon need. + These sessions are generally at GAMA + 1-2 half day sessions may be done from home via telemedicine dependent on clinical availability, personal preference, and insurance guidelines for telemedicine coverage 10-20%Office Practice (Non-Face to Face Time) + Assist office nursing and secretarial staff in phone management and triage of patients. + In basket management of provider’s personal in basket and those of any other provider who is unavailable + Complete clinical documentation, forms/paperwork requested by patients, prior authorization appeals, sign home care orders + Coordinate with other providers, home care and hospice teams, and pharmacies + Assist in development of practice, community outreach, educational materials, ect…. Duties may include presentations + Maintain professional credentials to allow for uninterrupted billing and clinical activity. 10-20% Home Visit Practice + See 2-4 patients in each half-day care session in the community (generally patient’s own home but may be in other private dwelling including comfort care home or a family member/caregiver’s home). 2 sessions per week depending upon need. **Salary Range:** $98,000.00 - $130,083.20 annually _The referenced pay range represents the minimum and maximum compensation for this job.
