SUMMARY

Plans, supervises, evaluates, and coordinates the delivery of home health services to participants in collaboration with the Director and in accordance with the policies and procedures of Rochester Regional Health and ElderONE.  Develops nursing policies and standards for the delivery of care.   Focuses on customer service as it relates to patients, visitors, family members, staff and physicians including working collaboratively with other disciplines to ensure an efficient, team approach to the delivery of patient care.  Assures compliance with NYSDOH, LHCSA and PACE regulatory requirements.  Responsible for the direct supervision of RNs, LPNs and Home Health Aides in accordance with Home Care standards.  Provides guidance for staff members in developing appropriate expectations and outcomes.

SCHEDULE: Monday-Friday, 8:00am - 4:30pm

RESPONSIBILITIES

Management of human, fiscal, educational and communication resource programs in the coordination and facilitation of homecare services through ElderONE.Plans, evaluates, and coordinates the delivery of home health to the participants. Ensures implementation of established care plans and related care activities. May participate in the development of the care plans.Maintains appropriate staffing to assure timeliness and quality of the services provided.Ensures competency of staff through monitoring clinical competencies as well as completed competencies for paraprofessional staff.  Evaluates staff using established productivity and qualitative standards.Assists in the recruitment, interviewing, selection, evaluation, performance management and separation process in accordance with personnel policies.Ensures timeliness and quality of documentation to meet all JCAHO, NYSDOH, and CMS standards.Develops, justifies and implements, in collaboration with the Director, the long-term staffing pattern, personnel complement needs and recruitment plan. Meets regularly 1:1 with staff in addition to conducting staff meetings.Accountable for monitoring fiscal data relevant to materials/human resources. Adjusts allocation in response to needs. Performs other duties as assigned.​​​

REQUIRED QUALIFICATIONS

Associate degree in Nursing required. 3-5 years of management or supervisory experience required.Minimum 2 years of certified homecare or licensed home care service agency experience as an RN.Working knowledge of NYSDOH LHCSA or CHHA regulations and Federal Conditions of Participation.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$85,000.00 - $135,000.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
