Job Description
A behavioral health client of ours in Pleasanton, CA is seeking a Medical Records Clerk to support the management, protection, and maintenance of sensitive behavioral health patient records. This role plays a critical part in ensuring compliance with HIPAA, and applicable county/state regulations while supporting clinical, administrative, and compliance teams.
The Medical Records Clerk will work onsite, four days per week (32 hours, 8 hours a day Mon-Thurs), during standard business hours under a 5‑month contract.
Key Responsibilities:
• Maintain, organize, and manage behavioral health and mental health patient records, mostly electronic and some paper-based
• Scan, upload, index, and file clinical documentation that they get from faxing or mailing into the electronic health record (EHR) system (Valent)
• Ensure medical records include required documentation such as:
○ Intake assessments
○ Psychiatric and psychological evaluations
○ Treatment plans
○ Progress notes
○ Discharge summaries
• Process Release of Information (ROI) requests in compliance with HIPAA
• Verify authorizations prior to releasing records and ensure appropriate redaction of restricted information
• Track missing, incomplete, or unsigned documentation and follow up with clinical staff as needed
• Assist with record preparation for audits, quality reviews, legal requests, and inspections
• Maintain strict confidentiality of patient information at all times
• Support record retention and destruction schedules in accordance with regulatory requirements
• Work collaboratively with clinicians, administrative staff, and compliance teams
The payrate range starts at $22/hr, and has flexibility depending on qualifying skills and experience.

Skills and Requirements
• 2+ year of experience in medical records, health information management, or a related role.
• Proficiency with Epic or related EHR/EMR systems.
• Experience processing Release of Information (ROI) requests
• Proficiency with HIPAA.
• Associate's Degree required; Bachelor's degree highly preferred.
• Related certifications (HIPAA, Confidentiality, etc)
Ability to handle highly sensitive and confidential information professionally; Strong attention to detail and organizational skills • Prior experience in behavioral health, mental health, psychiatric, or substance use disorder settings strongly preferred.
