Job Summary
As the MDS Coordinator at Vantage at West Springfield, you will oversee the assessment process to ensure accuracy, compliance, and appropriate reimbursement. This role is critical to clinical documentation integrity and interdisciplinary coordination.

Responsibilities

·        Complete MDS assessments and care plans

·        Maintain assessment schedules and calendars

·        Audit clinical documentation

·        Communicate issues to DON

·        Maintain confidentiality

Qualifications

Skilled nursing facility experience required

Pay Range: $45,000/year - $65,000/year
