**Job Summary and Responsibilities** As a Manager, Care Coordination, you will oversee and lead the daily operations of the Care Coordination department, ensuring high-quality, patient-centered care transitions. Every day you will manage and mentor Care Coordination RNs, monitor performance, ensure regulatory compliance, and collaborate with interdisciplinary teams to optimize patient flow and effectiveness. To be successful, you will demonstrate strong leadership, strategic planning, and clinical expertise, driving continuous improvement and optimal patient care coordination for organizational goals. + Manages areas including organizing and executing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction), and reports needs within the Medical Center and Dignity Health, as well as government and regulatory reporting. + Implements Care Coordination activities according to the needs, requirements, and policies of the Medical Center, the affiliated medical groups and health plans, Dignity Health, any Federal and State agencies, and according to standard practices of the professions under the manager’s accountability + Consults and collaborates with other managers, physicians, administration, and community based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature. + Oversees the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care. **Job Requirements** Required + Bachelors Other Nursing or experience equivalent and three (3) years experience in Care Coordination or Case Management. , upon hire + Registered Nurse: NV, upon hire and Preferred + Certified Case Manager, upon hire **Where You'll Work** As the community’s only not-for-profit, faith-based healthcare system; Dignity Health Nevada has been guided by the vision and core values of the Adrian Dominican Sisters for more than 70 years.
