Job Title: Licensed Vocational Nurse (LVN)

Job Description

As an LVN, you will coordinate and administer medical services to patients, focusing on assessing, screening, educating, and treating patient conditions.

Responsibilities

Perform LVN duties such as patient intakes, preparing rooms, rooming patients, taking medical histories, performing patient screenings, recording vital signs, and taking temperatures.Adhere to standardized clinic policies, standard operating procedures, and protocols.Promote general wellbeing through health education and encouragement.Coordinate care and schedule appointments for home health, medical specialists, occupational or physical therapy, dental, and primary care.Maintain accurate records for reporting to various funding and inspecting agencies.Ensure regulatory compliance and inspection readiness by maintaining files.Handle medication intake, verification, reconciliation, logging, audits, and medication calls, supporting patients in understanding their doctor’s orders and prescriptions.Direct shift work and support Clinical Support Specialists (CSS) with medication calls, patient concerns, needs, and changes in medical symptoms.Be available for occasional overtime, covering day, evening, or night shifts as needed.Support training of new hires as part of shift duties.Participate in peer reviews of patient medical charts.Work with patients experiencing homelessness and co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders in a high-acuity care facility requiring 24/7 patient support and monitoring.

Essential Skills

Proficient in using electronic medical records (EMR) systems.Familiarity with medical terminology and patient care practices.Experience in behavioral health settings.

Additional Skills & Qualifications

Current CA LVN License.2+ years of nursing experience within Behavioral Health or Psych.Ability to work collaboratively within a multidisciplinary team.

Schedule

Full-time Day Shift (7am to 3:30pm)Full-time Swing Shift (3:00pm to 11:30pm)

Work Environment

The position requires working in a high-acuity care facility that provides 24/7 patient support and monitoring. The role may involve occasional overtime and shift work, including day, evening, or night shifts as needed. Employees will benefit from medical and dental coverage, as well as vacation time.

Pay and Benefits

The pay range for this position is $85000.00 - $90000.00/yr.
