This is a unique opportunity to provide "total care" for an individual with intellectual and developmental disabilities (I/DD) in a home-based setting. You will be the clinical cornerstone for this family, providing the professional support necessary to ensure the client’s safety and the family's peace of mind. Clinical Responsibilities (Medical Acuity) This role is for a "total care" client with high medical acuity. Your expertise will be vital in the following areas: + Specialized Feeding: Manage and administer nutrition and medications via feeding tube. + Direct Nursing Care: Provide comprehensive support, including monitoring vitals, mobility assistance, and personal care. + Medication Administration: Ensure accurate delivery of all medications in compliance with NJ state regulations. + Care Coordination: Collaborate with the family and care teams to implement the Individualized Support Plan (ISP). + Documentation: Maintain precise, timely clinical records of all treatments and interventions.
