Job Description
Coordinate and provide Medicare Wellness Visits to eligible patients in the primary care practice. This role involves utilizing Excel spreadsheets to track patients due for visits, scheduling these patients, and tracking completed visits. The position requires collaboration with medical assistants and other practice team members to review patient lists, quality measures, and coordinate care for those in need of Medicare Wellness Visits or identified as having quality gaps.
Responsibilities
+ Coordinate Medicare Wellness Visits and track patient progress using Excel.
+ Call and schedule patients for visits.
+ Collaborate with the medical assistant and practice team members.
+ Attend Care Management Meetings, Primary Care Collaborative meetings, and educational sessions.
+ Develop personalized wellness plans for patients with realistic goals and actions.
+ Educate and support patients in making informed health choices.
+ Promote patient engagement and compliance with appointments and treatment recommendations.
+ Complete all tracking and reporting requirements for outcomes and evaluation.
+ Perform phone triage, patient scheduling, pharmacy calls, and authorize refills.
+ Respond to life-threatening situations based on nursing standards and protocols.
+ Prepare exam rooms, restock supplies, sterilize instruments, and maintain equipment.
+ Document all nursing interventions in the electronic medical record.
+ Obtain medical records from various sources prior to patient visits.
+ Respond to patient, family, and healthcare provider inquiries professionally and promptly.
+ Follow infection control and patient confidentiality protocols.
+ Complete annual recertification for OSHA, Compliance, and maintain current CPR certification and nursing license.
Essential Skills
+ Must be a licensed LPN in Virginia.
+ One year of LPN experience preferred.
+ Completion of BLS class and re-certification every two years.
+ Ability to make professional nursing decisions within the scope of practice.
+ Possess complex patient assessment and management skills.
+ Basic knowledge of educational behavioral approaches to change behavior.
+ Professional interaction skills with physicians, patients, and healthcare personnel.
+ Experience with Epic, triage, managed care, and electronic health records.
Additional Skills & Qualifications
+ Provide care plans from visits including medication lists and individualized patient goals.
+ Conduct diabetes education and teach self-glucose monitoring and insulin administration.
+ Coordinate transitions of care from hospital to long-term care.
+ Communicate effectively with continuity providers.
Work Environment
Located at Portsmouth Family Medicine in Portsmouth, VA. The position follows a Monday to Friday schedule from 8 AM to 5 PM with a one-hour lunch break. The environment is supportive of learning and professional growth, offering exposure to a variety of illnesses and treatments within a family medicine department.

This is a Contract to Hire position based out of Portsmouth, VA.
Pay and Benefits
The pay range for this position is $27.00 - $29.00/hr.
