Valley Health Systems is seeking a passionate and dedicated Social Worker to join our Center for Senior Wellness. What You’ll Do

Develop and lead our Dementia Support Program, including patient enrollment, caregiver education, behavior-management strategies, and resource coordination.Host a Monthly Dementia Support Group and a Monthly Parkinson’s Support Group, creating safe and supportive spaces for patients and caregivers.Provide individual and group caregiver support sessions to help families navigate challenges confidently.Conduct Advance Care Planning (ACP) sessions for patients and families.Address Social Determinants of Health, such as transportation barriers, caregiver shortages, guardianship concerns, and nursing home or assisted-living placement.Support patients and families through end-of-life decision-making and hospice referrals with empathy and clarity.Collaborate closely with an interdisciplinary team to ensure holistic, patient-centered care.

What You Bring

Master’s Degree in Social Work.Active WV LGSW, LCSW, or LICSW.Supervision offered to LGSW to get fully licensed.Experience with geriatrics, dementia, Parkinson’s, or caregiver support preferred.Excellent communication skills, emotional intelligence, and a passion for serving seniors.
