JOB DESCRIPTION Job Summary
Provides support for justice liaison activities. Responsible for collaboratively with justice system agencies, ensuring members receive care coordination and appropriate access to eligible and necessary services. Represents as single point-of-contact and ambassador for members entering and exiting the justice system. Contributes to overarching strategy to provide quality and cost-effective member care.
Essential Job Duties
• Communicates and collaborates with justice system (i.e. jails, sheriff’s departments, correctional health services and others) on behalf of members.
• Acts as the interagency liaison with department of corrections (county jails, sheriff’s office, correctional health services, department of juvenile corrections, office of courts and probation departments).
• Ensures appropriate transitions both in and out of the justice system, including care coordination and completion of documentation within the member record.
• Responsible for justice system reach-in initiatives, monthly or as needed; reports and develops a justice involved strategic plan.
• Interfaces with the court coordinator regarding drug court, behavioral health court and criminal proceedings for members.
• Shares information that assists the clinical team in developing treatment plans that incorporate community release conditions.
• Coordinates with providers, including delegated providers to ensure appropriate access to care and follow-up.
• Develops and initiates collaborative interagency protocols and memorandum of understanding (MOU) agreements within the court and justice systems.
• Provides coverage in the absence of the court coordinator, including attending and participating in court dispositions, coordinating court-ordered evaluations and treatments, and communicates follow-up needs with internal care coordination staff. groups as assigned.
• Local travel may be required (based upon state/contractual requirements).
Required Qualifications
• At least 2 years experience working within the criminal justice system, or equivalent combination of relevant education and experience.
• Demonstrated knowledge of community resources specific to the justice system and jail transitions.
• Ability to prioritize and manage multiple deadlines.
• Excellent organizational skills and detail-orientation.
• Ability to collaborate successfully with internal and external stakeholders.
• Strong problem-solving and critical-thinking skills.
• Experience in conflict management in the work setting, preferably health care.
• Excellent verbal and written communication skills.
• Microsoft Office suite/applicable software program(s) proficiency.
+ BA or BS in any social/behavioral sciences or related field from an accredited institution; a
+ A period of at least one (1) year working directly with the targeted population in performing case management services, including experience with the justice‑involved population or other case management experience.

Pay Range: $22.8 - $46.81 / HOURLY
