Salary Range $29.00 - $29.00 Hourly Position Type Full Time Education Level High School Category Nonprofit - Social Services Description Position Summary: The Lead Case Manager oversees the day-to-day operations of the housing program and ensures all program activities are implemented in compliance with HUD regulations, contract requirements, and organizational policies. This role functions as the primary program operations lead, responsible for coordinating service delivery, monitoring unit and program compliance, managing reporting requirements, resolving client-related issues and maintaining a caseload. Essential Job Functions: + Oversight of day-to day operations of the program + Intake and assessment of incoming participants to determine eligibility and appropriate services. + Maintain a caseload of 7 Families/Individuals. + Make referrals and appointments with appropriate agency, services and/or case managers. + Maintain client files according to company policy and contract requirements. + Conduct regular program file audits to ensure compliance with HUD, contract, and organizational documentation requirements. + Oversee unit compliance and ensure all annual inspections are completed timely and in full compliance with HUD standards. + Heavy documentation; HMIS data entry + Prepare weekly, monthly and annual reports. + Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested. + Provide all services in linguistically and culturally appropriate manner, including use of interpreters for those with limited English proficiency + Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and + Perform any other duties as assigned. Qualifications Education: + Graduation with a two-year degree in Psychology, Social Work, Community and Human Services, Rehabilitation Counseling or a related field is preferred; and + High School diploma or equivalent is required. Essential Qualifications: + Two (2) Years Case Management in Permanent Supportive Housing setting preferred; + Excellent communication skills; + Good computer skills; + Good documentation skills; + Be able to multi-task; and + Valid driver’s license + Must maintain and execute confidential information + Demonstrated ability to work with diverse communities. + CPR Certification, First Aid Certification, and an Annual Tuberculosis test Working Conditions: + Work is primarily performed in an office setting; + Some travel is required; and + Some outdoor activities may be required. Physical Demands: + Occasional lifting of more than 10 pounds; + Sitting, standing, walking, reaching are performed in the normal course of the position; and + The normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or potential exposure may be required as a condition of employment.
