Position Summary:
The Intake Coordinator will provide information to outside sources on WellLife Network programs and availability. Screen potential residents for program admission. Assist in admission procedures.  Prepare outreach reports for various regulatory agencies. Develop and conduct efforts to identify new referral sources. Participate in outside meetings and presentations as needed.

Essential Responsibilities:

Handle telephone inquiries requesting information about WellLife Network Programs; determine the type of service that is being requested and refer to appropriate internal source or appropriate outside agency. Attend CPOA and SPA meetings as scheduled to maintain current information on housing related matters. Obtain and review pertinent data and history on applicants to be considered and admitted to WellLife Network programs. Determine appropriateness of the referral for a specific residence or program. Schedule all intake and admission appointments; including staff coverage. Screen potential tenants and make admission determination based on verbal and written material. Ensure clinical hookups and day program plans for prospective tenants have been made prior to admission. Maintain regular communication with assigned staff at residential housing facilities to ensure a collaborative process. Ensure that physicians authorization is completed and in participants file, as well as all related consents. Follow-up on all accepted referrals until they are placed in WellLife Network or in another agency. Set up initial screening and program trial visits. Complete Outreach as directed. Position requires some field work (vehicle required) and 2 days weekly in office. Other days may work remote or as determined by Director of Intake Services.

Other Responsibilities:

Make community contacts with possible referral sources, clinics, in or out patient departments, psychiatrist, day programs, etc. Make presentations at various outside agencies on an as needed basis. Maintain confidentiality at all times. Work locations and schedules are subject to change.

Masters degree in Social Sciences and two (2) years’ experience in mental health or Bachelors degree, with 5 years’ experience in Mental Health. Valid New York State driver’s license and vehicle required.
