Intake and Enrollment Manager

Summary:
The Intake and Enrollment Manager will ensure optimal occupancy across all CDS Monarch programs, including residential services, day habilitation, community-based programs, and new housing developments serving individuals with intellectual and developmental disabilities and seniors. The Intake and Enrollment Manager will oversee the screening and processing of all applications, ensuring completeness, accuracy, and compliance with agency policies and regulatory standards. The Intake and Enrollment Manager will collaborate with Care Managers, DDRO Vacancy Management, and external partner agencies to market programs and coordinate placements.
Essential Job Functions:
+ Provide direct supervision and leadership to Intake Coordinators and Guardianship Program, ensuring adherence to agency policies and procedures to facilitate timely and efficient enrollment processes.
+ Chair and manage the Agency Intake Committee, including establishing committee membership, setting agendas, and conducting bi‑weekly meetings to review and approve admissions in accordance with agency guidelines.
+ Oversee the preparation, accuracy, and timely submission of all required admission, transfer, and discharge documentation, including DDP‑1, DDP‑2 Participant Change Forms, and authorization request forms.
+ Monitor intake applications and tracks progress throughout the enrollment lifecycle, including conducting pre‑admission meetings and ensuring completion of all required documentation for new admissions and internal transfers.
+ Ensure program occupancy targets and required fill rates are achieved and maintained across all services
+ Establish and implement growth strategies for Day Habilitation and Community‑based programs.
+ Conduct program site tours for prospective individuals and families, as needed, to support informed placement decisions.
+ Develop, maintain, and update intake and discharge policies and procedures for all applicable programs, including Residential, Day Habilitation, Community, Senior Living, and Monarch housing programs.
+ Represent the agency in monthly Vacancy Management meetings with the DDRO and serve as liaison regarding vacancy coordination, overcapacity placements, temporary placements, and residential placement planning.
+ Maintain waiting lists for individuals with intellectual and developmental disabilities seeking CDS Monarch housing and serves as liaison with property management for housing‑related coordination.
+ Oversee Corporate Guardianship services for assigned individuals, ensuring compliance with applicable laws, regulations, and agency standards.
+ Assist with the establishment and maintenance of administrative records and charts related to intake, enrollment, and discharge activities.
+ Perform other related duties as assigned by the supervisor to support agency operations and strategic goals.
Knowledge, Skills, and Abilities
+ Must possess a valid NYS driver’s license and meet CDS Monarch Vehicle Operator Requirements.
+ Ability to work independently and motivate others.
+ Ability to communicate effectively, both orally and in writing.
Education and Experience:
+ Bachelor’s degree in education, Social Work, Psychology, or Human Services related field with at least two years’ experience with developmental or intellectual disabilities.
+ NYS license in Social Work preferred
+ Experience in leadership or supervisory experience preferred.
+ Previous experience with intake preferred.
All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Executive Vice President, in collaboration with Human Resources.

+ Ability to turn/twist upper body
+ Ability to use hand for repetitive action and fine manipulating for the purpose of keyboarding
