In-home Peritoneal Dialysis/ Home Hemodialysis Training Nurse

The In-Home Peritoneal Dialysis Training Nurse (also known as PD Home Therapy RN or Home Training RN) is a specialized Registered Nurse responsible for training patients and their care partners to safely and independently perform peritoneal dialysis (PD) at home. This role emphasizes patient education, hands-on training, ongoing support, and home visits to ensure compliance with treatment protocols, prevent complications like peritonitis, and promote quality of life for end-stage renal disease (ESRD) patients.

 

Key Responsibilities Patient & Care Partner Training (Primary Focus): Deliver comprehensive, hands-on PD training (e.g., CAPD or APD) over 9–12 weeks in clinical settings and in-home. Teach aseptic techniques, catheter care, solution exchanges, machine operation, troubleshooting, and infection prevention. Adapt teaching to diverse learning styles, including family members or caregivers. Home Assessments & Follow-Up: Conduct initial and annual home visits to evaluate environment suitability, supplies, and technique. Perform monthly clinics, telephonic support, and on-call troubleshooting for complications (e.g., peritonitis, exitsite issues). Case Management: Monitor patient progress: Labs, vital signs, adequacy testing, and adherence. Collaborate with MDT (nephrologists, dietitians, social workers) for individualized care plans. Quality & Compliance: Ensure regulatory compliance (federal/state RN qualifications for home dialysis). Document thoroughly in EMR; report incidents.   Qualifications Active RN license. RN with experience in PD training. Minimum of one-year nursing experience, with expertise in hemodialysis and peritoneal dialysis. At least six months of general dialysis experience. CDN (Certified Dialysis Nurse) preferred. BLS/ACLS required.
