This pharmacist role collaborates closely with physicians, nurses, and interdisciplinary teams to optimize therapeutic outcomes, prevent medication errors, and support clinical decision‑making. The Hospital Pharmacist also plays a key role in sterile compounding, medication reconciliation, and compliance with hospital and regulatory standards. Here are some of the benefits of working at Prime Healthcare: + Health, dental, and vision insurance options + Paid vacation, sick time, and holidays + Bereavement leave, FMLA, and other leave options + Employer 401K options + Tuition reimbursement options + Life, disability, and other insurance options + Many other amazing benefits Full benefits at Prime Healthcare:https://www.primehealthcare.com/careers/benefits/ Responsibilities Essential Duties and Responsibilities (includes, but are not limited to): + Clinical Medication Review & Verification :Evaluate, verify, and optimize inpatient medication orders for safety, accuracy, and clinical appropriateness. + Interdisciplinary Collaboration & Rounding: Participate: in patient care rounds and partner with physicians, nurses, and care teams to support evidence-based medication therapy decisions. + Medication Preparation & Distribution Oversight: Ensure accurate dispensing and oversee sterile and non-sterile compounding processes in compliance with USP 797/800 standards. + Medication Reconciliation & Transitions of Care: Perform medication reconciliation during admission, transfer, and discharge to prevent errors and support continuity of care. + Patient & Staff Education: Provide medication education to patients when needed and serve as a drug-information resource for clinicians and hospital staff. + Quality, Safety & Compliance: Support medication safety initiatives, report adverse drug events, maintain regulatory compliance, and uphold hospital policies/protocols. + Inventory & Operational Support: Manage medication inventory, emergency supply readiness, and support efficient pharmacy operations. + Clinical Program Participation: Contribute to antimicrobial stewardship, pain management, or other hospital clinical initiatives as assigned. Qualifications Before we go any further, we do have some deal-breakers.
