Hospital At Home RNSUMMARY (Basic Purpose of the Job)Delivers quality care to patients through the nursing process of assessment, diagnosis, planning, implementation and evaluation within the home setting. Assures patient safety by executing appropriate policies. Serves as an advocate by providing emotional and informational support to patients and families and giving them an opportunity to participate in their plan of care and goal setting. Plans for providing traditional and individualized comfort measures. Works with other healthcare professionals to optimize patient outcomes and customer satisfaction. Educates patients about various medical conditions and provides advice and emotional support to patients' families. Contributes and supports the organization's readiness for various regulatory agencies, Environment of Care, Magnet and various certifications.MINIMUM REQUIREMENTS

Education:Associate's degree or graduate from an accredited school of nursing. Bachelor's of Science degree in nursing (BSN) preferred.

Experience:Three years experience in an acute care setting. Previous telemetry experience required.

Other Credentials:AHA ACLS or must obtain within 6 months of hire date,AHA BLS - Healthcare Provider,Drivers License,Registered Nurse – NJ or Registered Nurse – NLC multi-state

Requires NIH Stroke Scale Certification (or must obtain by end of orientation) AND 8 hours of Stroke related Continuing Nursing Education (CNE) contact hours annually if assigned to: Critical Care, Intermediate Care Unit, Emergency Department, Neuro Units, Cardiology Inpatient at Hopewell, Peds ED, PACU, Interventional Radiology, CNI, Observation, Hospital at Home

Hospital At Home Command Center RN

SUMMARY (Basic Purpose of the Job)Delivers high-quality acute care to patients receiving hospital-level services in their homes by remotely monitoring clinical status from Capital Health's centralized Command Center. Provides continuous virtual surveillance, clinical assessment, care coordination, education and timely intervention using advanced monitoring technologies and evidence-based nursing practices. Collaborates with clinicians, physicians, and interdisciplinary team members to ensure safe, seamless, and patient-centered care. Serves as an advocate by providing emotional and informational support to patients and families and giving them an opportunity to participate in their plan of care and goal setting. Hospital At Home Equipment Tech

SUMMARY (Basic Purpose of the Job)Provides support to department with routine, urgent, and complex needs to ensure quality of patient care. Oversees stock, preparation, setup, and maintenance of equipment and supplies in various locations where procedures are performed to ensure quality of patient care.MINIMUM REQUIREMENTS

Education:High school diploma or GED.

Experience:Previous work experience in a medical or healthcare setting preferred.

Other Credentials:AHA BLS. Non-Licensed must obtain before end of orientation period

Knowledge and Skills:Knowledge of basic medical terminology. Proficient in English. Verbal and written communication skills.

Special Training:CPR.
