Hospice Registered Nurse (RN)

Location: Richmond VA

Responsibilities:

Provide skilled nursing care and administer medications in patients' homes Complete assessments and develop care plans with the hospice team Monitor and report changes in patient condition Participate in interdisciplinary team (IDT) meetings Document all care using an electronic charting system Educate and support family members and caregivers Participate in on-call and weekend rotation as needed

Qualifications:

RN license in Virginia 1–2 years of RN experience required (hospice or home care preferred) Basic computer/iOS skills Strong communication, compassion, and teamwork skills
