Job SummaryThe Hospice Care Consultant 1 connects the agency with the community about needs. Responsibilities include educating health care providers and the community about available services. This aims to improve end-of-life quality while using human, financial, and equipment resources efficiently.
Essential Functions of the RoleProvides Hospice education at huddles to educate our healthcare partners on hospice services.Demonstrates the ability to drive community impact on hospice referrals. Develops key relationships with healthcare professionals, senior care facilities, and families.Develops meaningful relationships with internal key contacts and opinion leaders. This includes program directors, managers, nurse managers, office staff, and other key staff.Coordinates physician and provider educational visits to internal and external physicians and staff.Identifies and qualifies new sales opportunities and develops plans for introducing new solutions through collaborative relationships.Coordinates with clinical management and department staff to plan in-services and presentations. Addresses knowledge deficits in end-of-life care.Provides counsel to the home care/Hospice agencies regarding partnerships with our own Hospice agency to provide services throughout the community.Performs other position appropriate duties as required in a competent, professional, and courteous manner.Key Success FactorsTwo years’ experience in a home care or Hospice industry preferred.Excellent verbal and written communication skills.Ability to work and perform autonomously, minimal supervision.Proficient with Microsoft Office suites.Belonging StatementWe believe that all people should feel welcomed, valued and supported.
