Now Hiring: Speech Therapist, Home Health

PRN weekly visits | Territory: Florence and surrounding areas

The Speech Therapist will develop and implement care plans for their clients that coordinate the clients' home care services and follow physicians' orders. Ensuring that services are delivered according to licensing guidelines, professional medical standards, and agency policies and procedures.

** We can pay up to $100 for an Eval, up to $200 for a Start of Care, and up to $80 for a routine visit **

What You'll Do:

Assess and develop a program for therapies that meet individual client needsImplement the client's therapy planCoordinate therapy modifications to the client's plan of care with the Clinical Manager and client's physicianWork withthe Branch Manager and Clinical Manager to formulate or modify agency policies, procedures, and practices as neededEnsure effective coordination of client services through timely completion of required documentation and transfer of pertinent medical data to the client’s physician, Clinical Manager, and other caregiversMaintain accurate and comprehensive client medical dataInstruct clients, family members, and primary caregivers as needed to ensure the maintenance of or acquisition of optimal functioning level for each clientInvestigate and take appropriate actions on client or employee concerns and complaintsRoutinely review pertinent client medical data to determine the effectiveness of therapy services in reaching maximum rehab potential

What You'll Need:

Current Speech Therapy (ST) license in the state of the applicationValid CPRValid Drivers license and insurance

Preferred Qualifications:

Experience in Home Health Therapy and completion of OASIS documentation
