WHAT YOU WILL DO
• Manage and coordinate your own caseload
• Complete initial and ongoing patient assessments
• Develop and revise individualized plans of care
• Collaborate with physicians and interdisciplinary teams
• Document accurately and maintain HIPAA compliance
• Communicate changes in patient condition promptly
WHAT YOU NEED
• Active, unrestricted RN license
• At least 1 year of clinical nursing experience
• Willingness to travel within the branch area
• Reliable vehicle, valid driver’s license, current auto insurance
• Home Health experience preferred
• Experience with HomeCare HomeBase preferred
• Ability to push, pull, and lift 50 to 100 pounds as needed
