Registered Nurse (RN) — Home Health | PRN (Per Diem)

What You’ll Do

Perform start-of-care, resumption, recertification, and routine skilled nursing visits

Conduct comprehensive patient assessments (physical, psychosocial, and environmental)

Develop and manage individualized plans of care in collaboration with physicians and the interdisciplinary team

Provide patient and caregiver education on medications, disease management, and safety

Monitor patient progress and communicate condition changes to the physician and clinical leadership

Perform skilled nursing interventions (wound care, medication management, injections, post-hospital follow-up, etc.)

Complete timely and accurate documentation (OASIS and visit notes)

Coordinate services with therapists, aides, and other care team members

Maintain patient privacy and confidentiality in compliance with HIPAA

What You’ll Need

Current, unrestricted Registered Nurse (RN) license in the state

Minimum 1 year of clinical nursing experience (home health strongly preferred)

Strong assessment and patient education skills

Reliable transportation, valid driver’s license, and auto insurance

Ability to travel within the assigned coverage area

Comfortable working independently in a field-based setting

Experience with OASIS and electronic documentation (HomeCare HomeBase a plus)
