Bruce McCandless Veterans Community Living Center The Bruce McCandless Veterans Community Living Center (VCLC) at Florence offers holistic skilled nursing care for veterans, surviving spouses, and Gold Star parents, who need short-term rehabilitation, respite, or long-term care. The facility focuses on providing individualized, respectful care in a safe and comfortable environment. The VCLCs identifies and addresses residents’ diverse needs from admissions, transfers, and discharges, including social, emotional, psychological, and medical assistance. It advocates for residents, removes barriers, coordinates services, and addresses concerns. The unit also facilitates interactions between residents, families, staff, and external resources and ensures proper documentation for reimbursements. Description of Job We are seeking an exceptional individual interested interested in joining our nursing team! This position, under the direction of nursing or medical staff, provides direct, hands-on nursing care to residents in accordance with professional standards, regulatory as well as facility policy and procedure requirements. Responsibilities include assisting residents with activities of daily living (ADLs) such as but not limited to bathing, dressing, grooming, hygiene, skin care, range of motion, positioning, ambulation, transfers, toileting, feeding, and transportation. The role also involves performing basic tasks including vital signs, and weight/height measurements, documentation of care provided, and maintaining awareness of each resident’s physical and psychosocial needs and safety in a clean, infection-controlled environment. Additionally, as a Client Care Aide I, you can expect to: + This position provides direct, hands-on care to residents by assisting with ADLs such as bathing, dressing, grooming, personal hygiene, feeding and hydration, toileting, range of motion, ambulation, transferring, and transporting. The role involves maintaining infection control and safety through the use of universal precautions, proper lifting techniques, and appropriate use of equipment and safety devices such as transfer belts and lifts. Staff support residents’ independence by guiding and instructing them in self-care activities, arranging personal and environmental needs, and engaging them socially to enhance overall well-being. Communication with residents includes explaining care before, during, and after interactions, answering call lights, and collaborating with the interdisciplinary team to recommend routines, schedules, or care adjustments, including dietary modifications when necessary + Document observations and care provided on various residents in the electronic medical record (EMR) or paper records. Performs basic resident tasks such as weights, bowel and bladder incontinence, intake and outputs, vital signs, and records in the EHR and shift to shift report each shift + Transport residents for medical appointments, deliver blood draws to the lab, drive a resident to a doctor’s appointment, or to the hospital (if not transported by ambulance). etc. in state vehicles when the need arises.
