**Job Summary and Responsibilities** As a Medical Coder, you will ensure precise communication with insurance companies so that services are documented correctly and payments are processed efficiently. Every day you will accurately translate patients’ medical records into standardized codes for diagnoses and treatments. Using your expertise and training, you will ensure compliance with legal, regulatory, and organizational standards. To be successful in this role, you must combine accuracy and attention to detail with a strong knowledge of coding standards and healthcare regulations. Clear communication with providers and staff, along with efficient management of records, ensures claims are processed correctly and on time. + Daily Operations and Procedures + Retrieves and reconciles daily discharged records from multiple areas within the hospital + Technically evaluates and preps documents for the scanning process according to defined guidelines with a 95% quality metric and meets a minimum productivity standard of 4 inches/hour + Scans documents in a timely manner while verifying quality and potential scanning hazards according to defined guidelines and meets a minimum productivity standard of 1200 pages/hr + Maintains scanner and performs scanner maintenance processes according to defined guidelines + Performs Quality Control and Validation processes to ensure all documents are assigned the appropriate document name and all documents are assigned to the correct patient encounter with a minimum 95% quality metric and meets a minimum productivity standard of 800 pages/hour quality control and 1000 pages/hour validation + Reviews discharged medical record documentation to ensure record completeness according to hospital and departmental guidelines + Accurately assigns document deficiencies to responsible provider according to hospital rules and regulations and departmental guidelines + Interacts and communicates with physicians to assist with deficient and delinquent medical record completion + Faxes PHI for continuum of care with 100% accuracy following HIPAA guidelines, hospital policies, and professional standards + Coordinates work to achieve functional area goals and meets productivity and quality expectations on a day to day basis + Assists hospital staff, medical staff, public, and patients + Orders medical records from off-site vendor according to defined guidelines + Accepts and carries out other miscellaneous duties and responsibilities as required for maintaining the services provided by Health Information Management + Accurately records time worked and adhere to the Dignity Health Timekeeping policy + Provides Administrative Support + Able to respond to customer needs via phone or walk-ins regarding HIM Department + Efficiently retrieves patient records from the electronic medical record system + Supports both internal and external department request for records + Updates suspension list daily as physicians complete required charts + Compliance with Policy and Procedures + Maintains a professional and safe work environment + Complies with safety, infection control, Dignity Health Integrity Program, and other organizational policies (90-day evals- department specific orientation checklists completed; annual evals -annual competency checklists completed) + Complies with the organization's dress code **Job Requirements** Required + High School Graduate General Studies and Medical record experience., upon hire or + High School GED General Studies and Medical record experience., upon hire and + None, upon hire Preferred + Completion of Medical Terminology Course, upon hire **Where You'll Work** Marian Regional Medical Center, a 191-bed facility located in Santa Maria, California, is recognized as one of the Top 250 Hospitals in the Nation by Healthgrades and was awarded Best Maternity Care by Newsweek.
