Position Summary
The position is full time, 40 hours/week, in Roanoke or Norfolk, Virginia and candidate must travel to surrounding areas. Travel for onsite coaching events required. It requires proficiency in delivering onsite face-to-face coaching, group coaching and telephonic coaching, which will be evaluated during the interview process.
Work schedule:
Telephonic hours: M-F 9:00 am-5:30 pm EST with two evening rotations 12:30-9:00 ESTRotating late Friday and rotating 5 hour Saturday shift required per business needsApproximately 6 onsite visits per year; 2-3 consecutive days at each site, with potential for expansion of program. Daytime hours. May require overnight stay.Role Expectations:
The Wellness/Health Coach utilizes a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, and promote members in decisions related to achieving and maintaining optimal health status. The goal is to help members achieve a healthy lifestyle and behaviors that align these lifestyle behaviors with individual wellness goals.
The coach will conduct a comprehensive evaluation of member’s health status and risk with use of clinical tools and information along with data review.By assimilating health assessment data analysis, they will establish a plan of care based on member’s health status, risks, education level, and support needs.Motivational interviewing skills are required to ensure maximum member engagement and promote lifestyle and behavior changes to achieve optimum level of health.Utilizing interviewing and active listening skills you will provide coaching and support to empower the members to make ongoing independent healthy lifestyle choices.Develops and monitors established plans of care, in collaboration with the member to meet the member’s goals.Follows internal policy and procedure in compliance with regulatory and accreditation guidelines.

The typical pay range for this role is:
$46,988.00 - $102,000.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
