You’ll assess member needs, develop personalized care plans, and provide evidence-based coaching to support sustainable lifestyle changes. This role requires strong clinical judgment, communication skills, and a passion for helping others thrive. **Key Responsibilities** 🩺 **Assessment & Planning** + Conduct comprehensive nutritional assessments using clinical tools and data + Evaluate health literacy, technology access, and readiness for change + Develop individualized care plans based on health status, risks, and goals **Coaching & Engagement** + Use motivational interviewing and active listening to drive behavior change + Provide nutritional guidance and lifestyle coaching + Empower members to make informed decisions and engage with their providers **Monitoring & Documentation** + Track progress and adjust care plans as needed + Document all interactions in compliance with regulatory standards + Collaborate with multidisciplinary teams to ensure quality care **Required Qualifications** + Must reside within **commutable distance (≤45 minutes)** of High Point, NC + Willingness to represent A1A onsite for plan sponsor events and education + **Registered Dietitian (RD)** credential through the Commission on Dietetic Registration + **State licensure** as required (CVS Health supports multi-state licensure) + **5+ years** of experience in: + Medical nutrition therapy + Weight management + Disease management + Health behavior change + **5+ years** of coaching experience in a multidisciplinary setting + Strong computer skills (MS Teams, Outlook, Word, Excel) + Experience with motivational interviewing, mindfulness, and other coaching techniques **Preferred Qualifications** + **Certified Diabetes Care and Education Specialist (CDCES)** + Experience in managed care organizations + Proven success in facilitating positive behavioral change **Education** **Bachelor’s degree** in Nutrition, Dietetics, or a related field (required) **Why Join Us?** At CVS Health, you’ll be part of a team that’s transforming healthcare. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
