Overview: As a Family Specialist, you will supervise and provide direct services, focusing on family engagement and support within a mobile treatment setting. You will work closely with clients with severe and persistent mental illness, helping them achieve stability and well-being through compassionate, client-centered care. Key Responsibilities: + Conduct initial assessments and follow-up with new referrals within established timeframes. + Maintain accurate case records and documentation, including treatment plans, progress notes, and discharge summaries. + Collaborate with clients to develop safety plans, crisis interventions, and wellness strategies. + Facilitate group sessions and support individual treatment goals. + Assist clients with transportation, appointments, and community resources. + Ensure compliance with all agency policies, state regulations, and funding requirements. + Utilize agency tools and technology to document and coordinate care effectively.
