Family Medicine Physician – Integrated Public Health & Primary Care

Valley Health Systems, Inc.—one of the largest, multispecialty Federally Qualified Health Centers (FQHCs) in the nation—is seeking a mission-driven Family Medicine Physician to provide integrated primary care, infectious disease services, and community-based outreach for high-risk and underserved populations.

Position Summary

This physician will deliver a blend of clinic-based primary care and public-health-focused infectious disease services, along with community outreach in partnership with local and state public health agencies.

Key Responsibilities

Provide rapid access for infectious disease referrals, including HIV and Hepatitis C screening, intake, treatment initiation, and longitudinal management.Conduct outreach events in day report centers, recovery residences, homeless shelters, and other community settings, ensuring linkage to ongoing care.Deliver comprehensive primary care (acute, chronic, preventive) using evidence-based protocols and accurate documentation for quality/value-based reporting.Collaborate with care managers, CHWs, and specialty partners to ensure warm handoffs, continuity of care, and reduced ED utilization.Work closely with programs within Valley Health and community partners addressing social determinants of health and health disparities..

Qualifications

MD/DO; Board Certified/Eligible in Family Medicine.WV medical license (or ability to obtain).Experience or interest in infectious disease and public health preferred.Commitment to serving high-risk and vulnerable populations.
