The Director of Social Services will provide medically related social services to attain or maintain the highest practicable physical, mental, and psycho-social wellbeing of each patient. Responsibilities: + Oversee and supervise the social services department, ensuring the delivery of high-quality services to clients + Develop and implement case management services to all residents + Maintain contact with family (with resident's permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning + Maintain progress notes under federal and state guidelines for Long Term Care. + Ensure compliance with all regulatory requirements and standards in the provision of social services + Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents' individual needs. + Provide leadership and guidance to staff, including training, performance management, and professional development Requirements: + LCSW or CSW is required. Must be a licensed social worker. + BA from an approved Social Work school is required. + Subacute and geriatric experience + Minimum one years’ experience with seniors. In Supervisory capacity in a hospital, skilled nursing care facility, or other related medical facilities.
