Primary Purpose: 

 

Responsible for responding to needs of residents; functions in the role of Director of all nursing responsibilities for the Community. Responsible for providing emergency intervention to residents to facilitate the residents’ well-being and providing nursing intervention to facilitate the individual residents’ independence. Plans, organizes, develops, directs and evaluates the nursing service operation ensuring the highest degree of quality patient care. Ensures adherence to all applicable federal, state and local standards. 

 

Essential Job Functions 

Supervise all healthcare personnel to include hiring, training, evaluating and counseling within established policies and procedures. 

Supervise medication room, the LPN, Nursing Assistant, Medication Assistant/CMA, and Health Services Associate. 

Plan, implement, supervise, and schedule nursing services. 

Provide intervention to respond to emergencies and problems. 

Maintain medical charting and required documentation under scope state regulatory guidelines for appropriate licensing and corporate policies. 

Assess residents for change in level of care and coordinate billing for that level of care. 

Administer and coordinate medications of residents, serve as community’s Registered Nurse, order medication and refills. 

Work closely with residents and families to develop & implement appropriate care plans. 

Assist residents in medical appointments and coordinate required transportation. 

Participate in rotation of on-call duty. 

May supervise licensed nurses and nurse’s aides. 

Follow OSHA compliance and back support procedures. 

Supervise employee dress code, pager use, and name tags. 

Maintain contact with physicians to facilitate medication orders, supplies or transportation. 

Schedule staff and secures replacement for absences. 

Contact family emergency contacts as required. 

Assist in filling out Incident Reports for residents and staff. 

Promote the Mission, Vision and Values of Transforming Age in all areas of responsibility. 

Other duties as assigned. 

 

Required Skills/Knowledge/Abilities: 

The ability to speak, read and comprehend the English language 

Excellent written and communication skills 

Ability to work in a consulting role on a regional and facility level 

Strong leadership and teaching skills 

Proficiency with Microsoft Office Suite products 

 

 

Education/Equivalent Experience Required: 

Licensed as a Registered Nurse in state of employment. 

Minimum of two years’ experience in Nursing. Experience in Assisted Living, Memory Care is preferred. 

 

Special Requirements (if applicable): 

Coordinate resident care and clinic appointments 

Ability to be on-call as needed 

Maintain confidentiality of medical records. 

Doctor’s statement verifying free from communicable disease

Light work – exerts up to 20 lbs. of force occasionally, and/or a negligible amount of force frequently or constantly to move objects 

 

Visual acuity requirements, including color, depth perception and field of visions: 

Clerical/Administrative including the work of people who do analysis of data, work a computer terminal, and read 

 

Environmental factors: 

Worker is subject to environmental conditions with activities occurring both inside and outside of the community (nursing home visits). 

The worker is subject to hazards (hazardous waste). 

The worker is not substantially exposed to adverse environmental conditions.
