Direct Support Professional - Adult Services/Relief
About KDC  At KDC we are on a mission to empower and support all people to meet their full potential in a world where every individual and family thrives, no matter the circumstances.  About this role:   KDC seeks a relief Direct Support Professional to facilitate and provide both direct support and case management for adults with developmental disabilities. This hybrid, flexible role floats between Community-Based Day Services (CBDS) and Individual Supports programs, supporting individuals in their homes, the community, and program settings depending on daily needs. Using a team-based approach, this role promotes growth in independent choice and decision-making, community membership, household management, relationships, and personal accomplishments. Schedule: Flexible; weekday hours primarily, with Saturday or Sunday availability when needed + CBDS operates Monday-Friday, 8 am - 4 pm + IS operates Sunday-Saturday, 10 am - 6 pm Compensation: $22–$23/hour Location: Sandwich, MA and Yarmouth, MA (community-based; travel required) ​​​​​ We are looking for a team member with:  + High School Diploma or GED, required. + ASL fluency, strongly preferred. + Experience with and commitment to community-based supports for individuals with developmental disabilities. + Valid driver’s license and reliable vehicle, required. + Excellent organizational, interpersonal, and communication skills. + Ability to meet required competency levels in Microsoft Office. + Flexibility to float between programs and adapt to changing daily needs. The physical requirements for this role are:   + Extensive travel required. + Ability to meet with individuals within their homes. + Ability to sit for moderate time periods. + Frequent phone use required. + Ability to use computer. In this role, you’ll get to:  + Provide direct support to individuals in home, community, and program-based settings. + Deliver individual supports case management, including coordination of services and community resources. + Promote individual choice, self-expression, independence, and community inclusion based on quality service principles. + Provide direct skill training, supportive counseling, and crisis intervention as directed by individual service plans. + Facilitate assessment, implementation, and documentation of individual goals and treatment objectives. + Coordinate access to needed supports, including medical providers, community services, and other natural supports. + Support individuals in developing and maintaining meaningful relationships. + Provide transportation to appointments, activities, and community-based programming as appropriate. + Maintain accurate, timely documentation in compliance with KDC standards and funding agency requirements. + Collaborate with interdisciplinary teams, community providers, and medical professionals. + Participate in staff meetings, team meetings, and supervision. + Ensure all individuals are treated with dignity and respect and maintain confidentiality at all times. + Perform other job-related duties as assigned.
