Position Summary
This is a full-time field-based position in Ohio. This position requires routine travel up to 75% within the assigned region. The Community Health Worker (CHW) serves the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHWs help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position will work under the direction of a designated professional.
Fundamental Components:
Conduct home visits with Aetna members with behavioral health conditions to assist them with their care plan including identifying the options that best meet the needs of the member, adjusting tasks to fit with the member's culture, language, and religion, improving access to care, identifying behavioral health care needs, and reducing unnecessary PRTF hospitalizations to help the individual obtain positive behavioral health goals.Educate healthcare professionals and providers on opportunities for improving and understanding the social determinants of health that may be impacting member's health and treatment plan through various communication channels.Build and maintain positive working relationships with the Aetna members, providers, behavioral health case managers, agency representatives, supervisors, and office staff, from diverse cultural and socio-economic backgrounds.Work to reduce cultural and socio-economic barriers between clients and institutions.Assist members to identify socio-economic issues that affect their overall health and develop health/social management plans and goals.Identify gaps and opportunities to strengthen systems of care within the member's community and assists members in utilizing community services, including scheduling appointments with social services agencies, (including transportation vendors), and assisting with completion of applications for eligible programs.Assist with patient medication adherence by educating and instructing the member on current medication list, reviewing medications with member and assist in obtaining refills, and identifying alternatives if necessary.

The typical pay range for this role is:
$18.50 - $31.72This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
