Sentara Medical Group is seeking to hire an individual to join our team as a Community Health Worker-Community Care Centers

Position Location: Norfolk, VA (rotation between clinics and mobile bus)

Position Status: Full-time Days

Hours: Mon & Tue 8:00-5:00pm, Wed & Thurs 8:00-6:00pm, Fri 8:00-2:00pm,

2nd & 4th Saturdays 9:00-12:00 pm

Night or weekend flexibility is needed for occasional community events.

Overview

Works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.

Education

High School Diploma or Equivalent required

Certification/Licensure

Driver’s License-Certification-Other/National

Experience

Experience working in a community-based setting preferredExperience working in community events and organizing events is preferred.Knowledge of medical terminology is preferred.
