Comprehensive benefits are effective the first of the month following 30 days of employment. _________________________________________________________________________________ Duties and Responsibilities + Develop a person-centered treatment plan that focuses on addressing the needs of the client to include family engagement when applicable + Coordinate care for the client with family and other involved individuals, i.e. school personnel, probation officer, mentor, DHHS, medical personnel, etc + Provide individual, family therapy, and case management, as needed to each client/family on his/her caseload + Market and network with community partners and current/potential referral sources. Examples of this include but are not limited to attending community collaborative meetings, distributing marketing material, providing a description of services provided + Complete all service plans, progress notes, discharge summaries and all other necessary documentation in a timely manner + Represent Samaritas mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled + Monitor the quality of overall treatment services to clients on his/her caseload; and evaluate the adequacy of these services to meet treatment objectives + Participates in regularly scheduled staff meetings, therapist meetings, and any others as deemed necessary by the SUD Leadership + Represent Samaritas mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled + Responsible for timely Quality Assurance compliance as related to employee’s roles and functions Professional Growth + Possess a working knowledge of licensing and accreditation standards and work to assure compliance with such standards + Utilize educational opportunities and resources offered by the agency and through the profession for continued development of skills and clinical expertise, (conferences, trainings, peer supervision) _________________________________________________________________________________ Job Qualifications Education, Training, and Licensure/Certification + Master Degree in Social Work, Counseling or a related field, from an accredited institution + Two years of clinical experience working with SUD clientele preferred + Prior experience working with an interdisciplinary team preferred. Experience + Minimum one year SUD counseling experience preferred + Minimum experience working on a team preferred + Familiarity with SUD documentation preferred + Fully licensed clinician preferred.
