Home Health Clinical Team Manager (RN)

Valparaiso Office/ Full Time

Due to continued growth and a census exceeding 200+ patients, we are expanding leadership and adding a second Clinical Team Manager to support OASIS workflow, staff development, and patient care coordination.

What You’ll Do

As a Clinical Team Manager, you will lead the clinical team while ensuring safe, compliant, and high-quality patient care.

Clinical Leadership & Operations

Oversee patient care services and daily clinical operations

Assign clinicians, coordinate admissions and referrals, and ensure timely start of care

Ensure individualized patient care plans are developed, implemented, and updated

Review and approve Plans of Care for clinical appropriateness

Integrate physician orders and coordinate communication between physicians, patients, and care teams

Monitor and manage OASIS workflow and documentation quality

Team Support & Development

Coach, mentor, and support field clinicians

Conduct regular team meetings and provide clinical guidance

Participate in interviewing and hiring of clinical staff

Communicate performance metrics and operational needs with the Branch Director/Administrator

Patient Care & Quality

Coordinate services with providers, facilities, and vendors

Ensure compliance with regulatory and agency standards

Remain available for occasional field visits and clinical support when needed

What We’re Looking For

Active, unrestricted Registered Nurse (RN) license

Associate degree in Nursing (required)

2+ years of home health or hospice RN experience

Supervisory or leadership experience preferred

Knowledge of OASIS documentation

Comfortable with technology (Homecare Homebase, EMR systems, Microsoft Office)

Ability to travel within the coverage area

Additional Requirements

Reliable transportation, valid driver’s license, and current auto insurance

Ability to sit, stand, bend, and lift intermittently (up to 50 lbs)
