Clinical Team Manager (RN) – Home HealthElara Caring / $5K Sign On Bonus / Bloomington

We’re looking for a Clinical Team Manager (RN) who wants to mentor clinicians, improve patient outcomes, and help build a positive team culture.
This is a hands-on leadership role — not a desk job. You’ll guide and support field staff while overseeing quality patient care.
What You’ll DoSupervise and support RNs, LPNs, aides, and therapists
Oversee patient plans of care and coordinate services with physicians
Review documentation and OASIS for accuracy and compliance
Lead case conferences and provide clinical coaching
Assist with hiring, onboarding, and staff development
Step into the field when needed to support care

RequirementsActive RN license (state specific)
Associate Degree in Nursing (BSN preferred)
2+ years Home Health or Hospice RN experience
Supervisory/preceptor experience preferred
Reliable transportation and local travel
Must be able to lift, pull, and push 50-100 lbs
