Clinical Pharmacist - Outpatient Oncology - Full-Time - Morristown Medical Center

The Outpatient Oncology Clinical Pharmacist will provide support to patients, pharmacists, physicians, and nurses in the Carol G. Simon Cancer Center at Morristown Medical Center. This clinical pharmacist will collaborate with multidisciplinary care teams to optimize pharmacotherapy, create and implement clinical guidelines and protocols, and review pertinent monitoring parameters to ensure safety and appropriateness of oncologic treatment regimens.  Additionally, this clinical pharmacist will participate in system-wide workgroups to assess the cost-effectiveness of care and evaluate alternative treatment options to optimize efficiency of care for infusion center patients.   

Additional responsibilities include:

Provide comprehensive pharmaceutical care to oncology patients in the Carol G. Simon Cancer Center at Morristown Medical Center. Patient care responsibilities include: review and recommend optimal, evidence-based medication therapy; provide pharmacokinetic consultations as required; review laboratory parameters and assess appropriateness for administration of outpatient oncologic therapies; document changes in patient and pharmacy records and inform others of medication changes; prevent, detect, monitor, document, and report adverse drug reactions and medications errors Build, update, and validate Epic Beacon treatment and therapy plans and protocols Participate in interdisciplinary oncology leadership meetings, weighing in from the pharmacy perspective on guideline and protocol updates, incorporation of novel therapies, and process improvement opportunities Take part in oncology pharmacy meetings, as well as Pharmacy and Therapeutics Committee meetings as needed for formulary additions Cross-cover inpatient oncology clinical pharmacist colleague(s) as needed Conduct medication use evaluations and research projects in collaboration with other healthcare professionals with the goal of publication and/or presentation at local and/or national meetings Assess feasibility of incorporating novel treatment approaches for infusion center patients, and liaise with medication suppliers and manufacturers as needed to procure medications for infusion center patients  Serve as a preceptor and mentor for pharmacists, PGY1 pharmacy residents, and students
