The Clinical Pharmacy Specialist plays a vital role in contributing to the prevention and cure of cancer by providing safe, evidence-based, cost-effective, and patient-centered medication management. As an integral member of the Malignant Hematology interdisciplinary team, the Clinical Specialist offers clinical pharmacy support to healthcare professionals and ensures optimal patient outcomes by recommending, designing, implementing, monitoring, and modifying pharmaceutical care plans.
ResponsibilitiesProvide oversight for the safe and appropriate use of medication prescribing, dispensing, administering, and monitoring.Deliver clinical pharmaceutical care to optimize drug therapy for cancer patients through recommendation, implementation, monitoring, and modification of individualized treatment plans in collaboration with the healthcare team.Perform medication order review, clarification, and verification, assessing for completeness and appropriateness.Supervise accurate hazardous and non-hazardous drug preparation and distribution per policies and procedures.Process and double-check chemotherapy orders as necessary.Develop, interpret, and provide medication-related information and education for pharmacists, providers, nurses, and other healthcare personnel.Coordinate and provide medication therapy education and counseling to patients and caregivers.Precept pharmacy students, residents, and others in training.Participate in clinical research and projects.Provide leadership for developing, implementing, and maintaining medication-related guidelines, policies, and standards.Manage the medication use process to improve workflow, quality, and safety for patients and staff.Oversee the cost-effective use of medications to ensure appropriate utilization of resources and reduce financial risk.Essential SkillsDoctor of Pharmacy DegreeBoard of Pharmacy Specialties (BPS) certificationExperience in medication management, oncology, and malignant hematologyAbility to develop and implement pharmaceutical care plansKnowledge of medication safety, formulary management, and policy developmentAdditional Skills & QualificationsPGY-2 Residency in Hematology/OncologyExperience in chemotherapy order processingExperience in clinical researchSkill in education of patients, family members, healthcare providers, and pharmacy students and residentsWork EnvironmentThe work environment offers a supportive and collaborative atmosphere, emphasizing teamwork and patient-centered care.
