USGP is seeking a qualified and motivated Clinical Pharmacist to support the Medication Therapy Management (MTM) Department at the Naval Branch Health Clinic in Mayport, Florida. Degree: Doctor of Pharmacy (Pharm.D.)Education: Graduate from an ACPE-accredited college or universityResidency/Fellowship:Completion of a clinical pharmacy residency or fellowship accredited by ASHP or ACCP,OR documentation of equivalent clinical education and CME in clinical pharmacyExperience:Minimum of 12 months of experience within the past 3 years providing direct patient care in an ambulatory clinical pharmacy settingCertification:Current Basic Life Support (BLS) through the American Heart AssociationLicensure:Active, unrestricted license to practice as a pharmacist in any U.S. state or territoryImmunization Requirements (Must be current or willing to obtain):

Responsibilities include, but are not limited to:
Evaluate and manage patient pharmacotherapy in collaboration with medical staffInitiate, modify, or discontinue medications for chronic conditions (e.g., diabetes, hypertension, asthma) under clinical protocolsInterpret and evaluate lab results to support therapy decisionsOrder therapeutic nutrition products and adjust drug therapies based on patient responseReview patient profiles for interactions, contraindications, allergies, and therapeutic duplicationsProvide direct patient counseling on medication adherence, self-monitoring techniques, and compliance aidsUtilize EHR systems for medication documentation, tracking, and communicationAdminister or prescribe medications per established agreements and protocolsDocument and report adverse drug reactions (ADR) and medication errors in accordance with TJC or AAAHC standardsParticipate in medication utilization reviews (DUR), quality improvement projects, and staff training initiativesProvide clinical consultations, patient assessments, and support for disease state management and polypharmacyEducate patients and staff on medication therapies, safety, efficacy, and emerging pharmaceutical trendsSupport interdisciplinary health teams with therapeutic recommendations and contribute to care planningMaintain knowledge of pharmacogenomics, pharmacokinetics, and complex medication regimensIf certified, may perform auricular acupuncture as permitted by DHA and MTF policiesWhy Join USGP?
