MAJOR FUNCTION:
The Clinical Coordinator supports the day-to-day operations of the primary care office. This position combines clinical responsibilities with administrative tasks, ensuring that patient care is efficient, organized, and delivered in a professional manner. The Clinical Coordinator will work closely with healthcare providers, staff and patients to facilitate high-quality care and support a positive patient experience.
QUALIFICATIONS:
Education & Experience:
High School required.Minium of 2 years of previous primary care experience or related field required.Certification/ Licensure:
Current (LPN) license in the state of NJKnowledge & Skills:
Must have critical thinking skills and maintain confidentiality.Excellent organizational skills and ability to handle a variety of tasks simultaneously.Knowledge of medical terminology.Strong decision making and prioritization skills.Ability to work respectfully and professionally with the community, patients, families, and staff.Sympathetic, mature, responsible, and reliable.Strong patient engagement, interpersonal, and communication skills and ability to establish a therapeutic relationship with the patient.Position Responsibilities
Administrative Duties
• Ensure timely completion of paperwork and follow-up for high-priority patient needs,
including but not limited to: oxygen setup, DME requests, IV iron infusions, Prolia,
IUD/Nexplanon, Botox, and medical marijuana setups (where applicable).

• Coordinate and manage resident scheduling in collaboration with the Office Manager and
Program Director (where applicable).
Clinical Support
• Perform patient intake assessments, obtain vital signs, and prepare patients for
examinations and procedures.
• Collaborate with physicians and clinical staff to coordinate and implement patient care
plans.
• Provide patient education regarding treatment plans, medications, and follow-up care
instructions.
• Participate in daily huddles with providers, clinical staff, and the Manager to prepare for
the day's activities.
• Complete pre-visit chart preparation to ensure all necessary information and
documentation are in place before appointments.
• Monitor and manage clinical and provider inboxes, addressing patient inquiries and
assisting with follow-up actions as needed.
• Support direct patient care activities, including wound care, injections, assisting with in office procedures, and other clinical tasks as required.
• Oversee the daily nurse schedule.
• Maintain stock of medical supplies, vaccines, and equipment.
• Ensure compliance with infection control standards and promote a safe, clean
environment for patients and staff
Collaboration and Communication:
• Work closely with manager to develop and implement departmental goals and strategies.
• Maintain effective communication with physicians, specialists, and other health care
providers.
