Clinical Case Manager Behavioral Health - Work at Home

And we do it all with heart, each and every day. **Position Summary** Coordinates and manages comprehensive care by assessing individual needs, creating personalized care plans, and facilitating services across healthcare providers. Acts as a patient advocate to ensure clear communication and access to appropriate resources. + **Required Qualifications** + Active Independent Behavioral Health License in your state of residence. + Reside in Mountain Time Zone, or be able and willing to work Mountain Time Zone Hours of 8-430 MT. + Minimum of 3 years of post-master’s clinical experience in behavioral health. + Strong understanding of mental health and substance use disorders. + Proficiency in technology, including Electronic Medical Records (EMR), Microsoft Office Suite, and general computer applications. + Dedicated, private home workspace with reliable high-speed internet. + Remote position: Must work exclusively from your primary home residence; working from alternate locations is not permitted. **Preferred Qualifications** + Experience in **case management** and **discharge planning** . + Familiarity with **managed care systems** and processes. + Previous **remote or work-from-home experience** . + Skilled in **motivational interviewing** techniques to support patient engagement and behavior change. **Education** + Master’s degree in **Behavioral Health** , **Social Work** , or a closely related field. **Anticipated Weekly Hours** 40 **Time Type** Full time **Pay Range** The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
