SUMMARY
As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.
This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.

Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errandsCommunication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularlyDocumentation. Accurately and completely document client care on activity sheetCompliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirementsMinimum Qualifications:
Candidates must meet one of the following:
Active Home Health Aide (HHA) certification
HHA with a lapse of more than two years of employment (must complete written exam and 1-day HHA Skills Competency Class within 30 days of hire)
Certified Nursing Assistant (CNA) willing to complete a written exam and 1-day HHA Skills Competency Class within 30 days of hire
Out-of-state HHA or Nurse Aide training with documented competency evaluation
Nursing student (Fundamentals of Nursing completed within the past 24 months)
U.S. Military-trained medic or medical technician
Preferred Qualifications:
High school diploma or equivalent
1 year experience working with the frail elderly population
Compassionate, patient focused and dependable professional
Critical thinking skills, decisive judgment and the ability to work with minimal supervision
Exceptional documentation and communication skills
Ability to follow verbal and written instructions

It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
