Case Planner-Respite Job Details Job Location 1606 Sunset Ave - Utica, NY Position Type Full Time Salary Range $25.47 - $25.47 Hourly Travel Percentage Up to 25% Job Shift Day Description Basic Function/Overall Responsibilities: Assist with implementing individualized family assessment service plans and facilitate productive working relationships between families, agency personnel and other involved service providers. Flexible work schedule; non-traditional working hours required, on call as needed. Principal Assigned Responsibilities: + Assist families in identifying and obtaining appropriate clinical, recreational, financial, medical or other support services designed to fully integrate the family with community-based support; make referrals as necessary. + Support families with scheduled appointments, etc. and provide or arrange transportation when necessary. + Act as liaison between parents/families, Family Court, Probation, school officials, agency personnel. + Actively participate in program meetings. + Complete documentation per agency policy and procedure (e.g. log entries, mileage, medical, incident reports, intake.) + Attend agency in-service trainings, workshops and seminars as appropriate or required by supervisor. + Seek out outside trainings related to job duties / case specific needs, as approved by their supervisor. + Take part in Quality Improvement training and demonstrate commitment and active participation in the agency's ongoing Quality Improvement Program. + Serve as the contact point, advocate, and informational resource for enrolled youth, his/her family/caregiver(s), and the youth’s care team. + Develop positive reciprocal working relationships with other service providers/agencies through timely communication, inquiry, and follow-up regarding transitions in care and treatment recommendations. + Provide education and facilitate linkages to youth and family/caregiver(s) about relevant community resources. + Ensure compliance with agency and program policy and procedures. + Other associated tasks as deemed appropriate by supervisor. Qualifications Education and Experience Requirements: + Bachelor's degree, preferably in human services or a related field, required for positions with Uniform Case Record (UCR) documentation responsibilities. + Minimum of one-year successful human service experience in a behavioral health setting, preferably working with children.
